Check your contract which will tell you whether you signed up with AATakeCharge Milestone, LLC under the outcome-mileage plan or with AAATakeCharge under the outcome-only plan.

If you no longer have a copy of your contract, fill out the AATakeCharge Payment Request Form. Attach a note to the form telling us you are not sure you are submitting the correct payment request form. As long as you include copies of your pay stubs we will complete the other form for you if we discover you are with AAATakeCharge.
